
Community Integration Team
(CI Team)
This team focuses on assisting
individuals with disabilities who choose to move from institutions to their
own homes in the community; and on preventing unnecessary institutionalization of people with
disabilities who are currently living in the community.

CI Team's Vision: |
No one will have to live in an institution due to the lack of appropriate
supports and services in the community.

CI Team's
current priorities include: |
People with disabilities will receive the services and supports they need to live in the homes of their choice.*
People with developmental disabilities or traumatic brain injuries who wish to work in competitive employment rather than a sheltered workshop will receive the services and supports they need to realize their employment goals.*
People with mental illness will receive mental health services and supports in their community to avoid unnecessary inpatient treatment.

The Lt. Governor's Office on Aging is seeking
additional consumer participation on its Systems Transformation
Grant Advisory Committee and Work Groups. Consumers with
disabilities; seniors; family members; and, state and local
agency representatives serve on the Grant Advisory Committee
and five work groups (Developmental Disabilities,
Transportation, Information Technology, Waiting Lists, and
Evaluation). These groups meet on a quarterly basis to provide
input and oversight to the Systems Transformation Grant,
which was awarded to the Lt. Governor's Office on Aging for the
period October 1, 2005 - September 30, 2010.
