Community Integration Team

(CI Team)

This team focuses on assisting individuals with disabilities who choose to move from institutions to their own homes in the community; and on preventing unnecessary institutionalization of people with disabilities who are currently living in the community.

The Lt. Governor's Office on Aging is seeking additional consumer participation on its Systems Transformation Grant Advisory Committee and Work Groups. Consumers with disabilities; seniors; family members; and, state and local agency representatives serve on the Grant Advisory Committee and five work groups (Developmental Disabilities, Transportation, Information Technology, Waiting Lists, and Evaluation). These groups meet on a quarterly basis to provide input and oversight to the Systems Transformation Grant, which was awarded to the Lt. Governor's Office on Aging for the period October 1, 2005 - September 30, 2010.
